Applicants may apply for grants of up to $100,000 per year and may apply for one- or two-year grants.
||The Blue Cross and Blue Shield of Minnesota Foundation’s mission is to make a healthy difference in communities by advancing health equity and improving conditions where people live, learn, work and play.
Social determinants of health influence a full 40 percent of health outcomes. Focusing on the social determinants of health can reduce barriers and positively impact future health. This funding opportunity is
focused on the intersection of social connections and community safety — key social determinants of health.
When individuals and families feel secure and connected in their personal lives and in their community, they can thrive at home, school, and work and achieve positive health outcomes. The opposite is also true: violence, the fear of violence, chronic stress and isolation result in negative health consequences.
Through Healthy Connections we will work with our grantee partners to help create and nurture more connected, resilient and inclusive communities where all people have the support and resources to reach their full health potential. Communities that experience structural barriers to safety and connectedness include Native Americans, people of color, immigrants, refugees and people in rural communities.

1. Cover page including PI name, department, contact information, project title, any co-PI's name and department, and one paragraph project abstract.
2. Project summary (not more than three pages).
3. Biosketch of PI and any co-PI's (NIH or NSF format, or similar. Not more than five pages each).

Registration also provides information that helps the university office overseeing the nomination to determine the level of coordination needed and guide the review process.

- Once the internal deadline has passed, the status will change to “In Review” and proposals will undergo review.
- If there are multiple applicants, the office managing the nomination will determine the review criteria and coordinate the review process.
- Once review has been completed all parties will be notified via email.
- Nominees will be provided with a letter addressed to the sponsor from the OVPR indicating that they are the university's nominee. This letter should accompany the nominee's proposal.
- SPA is notified of the university's nominee(s) and the status is updated to “PI Selected”.
